Custom Home Health  programs emphasize a multi-discipline team approach for the patient’s needs and those of their caregiver.

Custom Home Health’s unique “Touch Health” is a hands-on team approach, with scheduled home visits from a registered nurse for regular evaluation, patient education, and care management. As frequently as needed, daily when necessary, Custom reaches out to their patients with a “touch” via telephone from a Cardiac Care Manager upon discharge from Custom Home Health.

The Custom Cardiac Care program awards every patient a Certificate of Completion at discharge. This certificate represents the increased knowledge and awareness of their cardiac condition. We have seen excellent results in patient care and patient discharge with our Touch Health program.
